2-2BViolation of Code: [511-6-1.03(5)(j)1&2] REPEAT: Observed an employee’s water bottle on the prep table next to the hand washing sink. Corrective Action: Told Person-In-Charge that water bottles can not be in the kitchen. Employees must use a single-service cup with a lid and a straw. Employee drinks must be placed away from food and food prep surfaces.  Repeat Violation.
4-2AViolation of Code: [511-6-1.04(4)(c)1(iv)] REPEAT: Observed bulk containers of flour, sugar, and pintoes stored uncovered on shelves. Corrective Action: Instructed Person-In-Charge that food containers must be kept covered at all times. Bulk food containers were closed with lids.  Corrected On-Site.  Repeat Violation.
4-2BViolation of Code: [511-6-1.05(7)(b)] REPEAT: Observed mold build-up on the inside flap of the ice machine. Corrective Action: Told Person-In-Charge to clean these items more frequently.  Repeat Violation.
4-2BViolation of Code: [511-6-1.05(6)(n)] Did not observe measurable sanitizer at the 3 compartment sink. Corrective Action: Instructed Person-In-Charge to change the sanitizer out with new.  New Violation.
5-1BViolation of Code: [511-6-1.04(5)(h)] Observed improper reheating for hot holding. Meatloaf in warmer cabinet temped at 2:45 PM was at 109°F; meatloaf was placed in cabinet at 11:45 AM. Person-In-Charge stated the meatloaf is removed from the walk-in freezer, placed in the warmer cabinet until needed, cooked in the oven for 5-10 minutes, and then placed on the buffet line. Corrective Action: Instructed Person-In-Charge that proper reheating procedure is to take the meatloaf from the walk-in freezer, cook in the oven to 165°F throughout, then place in the warmer cabinet until needed, and then place on the buffet line. The meatloaf was discarded.  Corrected On-Site.  New Violation.
6-1AViolation of Code: [511-6-1.04(6)(f)] REPEAT: Observed multiple time and temperature control for safety food items on the food bar cold held above 41°F (See Temps). Corrective Actions: Since bar had been restocked after lunch, Person-In-Charge removed the food to the walk-in cooler to cool down. Establishment will use Time as a Public Health Control for the items on the cold bar.  Corrected On-Site.  Repeat Violation.
6-2Violation of Code: [511-6-1.04(6)(g)] Observed potato salad and open container of milk in the walk-in cooler not labelled with dates of opening or preparation. Corrective Action: Instructed Person-In-Charge that food items made in the kitchen must be labelled with the name of the food and the date prepared, and food containers must be labelled with the date once they are opened.  New Violation.
14AViolation of Code: [511-6-1.04(4)(k)] REPEAT: Observed handles of scoops stored in the bulk containers of sugar and cornmeal touching the food. Corrective Action: Instructed Person-In-Charge that scoops must be stored so the handle does not touch the food.  Corrected On-Site.  Repeat Violation.
14BViolation of Code: [511-6-1.05(10)(a)] REPEAT: Observed multiple pans being wet stacked at the shelf across from the ice machine. Corrective Action: Told Person-In-Charge that pans have to be completely air dry before they can be stacked together. Air drying is the final step of the sanitization process.  Corrected On-Site.  Repeat Violation.
14CViolation of Code: [511-6-1.05(6)(r)] Observed cardboard lining the floor in front of the grill and in the walk-in cooler. Corrective Action: Instructed Person-In-Charge to remove the cardboard.  New Violation.
15AViolation of Code: [511-6-1.05(6)(a)] Observed torn gasket on the walk-in freezer and the cart next to the dish machine with a lot of rust. Corrective Action: Instructed Person-In-Charge to replace the freezer gasket and to replace the cart or scrape it and paint it.  New Violation.
17DViolation of Code: [511-6-1.07(4)(b)] REPEAT: Observed employee cellphone and charge cord stored on the prep table next to the aluminium foil. Corrective Action: Told Person-In-Charge that employees’ personal items must be in a designated area away from food and food contact surfaces.  Repeat Violation.
